Third person who died in Tooele County crash ID’d

TOOELE, Utah (AP) - Officials on Monday were investigating a weekend car crash in Tooele County that killed three people and injured six others.

The crash occurred Sunday afternoon when a minivan carrying two adults and five teenagers tried to change lanes on State Road 36 near Stansbury Park, the Utah Highway Patrol said.

The van struck another car, crossed a median and struck another vehicle traveling in the opposite direction.

The driver and passenger in that vehicle were killed. Troopers identified them Monday as Urban Montoya, 61, and Melodie Clement, 58, both of Salt Lake City.

A passenger in the minivan, Donnett McCollin, 43, from the Dugway area, was also killed.

McCollin's husband, the minivan's driver, was in critical condition Monday, Highway Patrol Sgt. Cory Nye said.

Four teenagers, ranging from 14 to 17, were thrown from the minivan onto the road in the crash. They, and one other teenager, were all in serious condition Monday, KSL-TV reported.

Troopers said most of the passengers in the minivan were not wearing seatbelts.

The seven adults and teenagers in the van were part of a youth church group from a chapel on Dugway Proving Ground, the U.S. Army base about 85 miles southwest of Salt Lake City.

The minivan was one of four vehicles headed to Salt Lake City for Christian music concert, said Dugway spokesman Al Vogel.

Vogel said those involved were parishioners who attend the nondenominational chapel on the base.

The base did not release additional details Monday about the individuals involved in the crash.

"This is a tragic event that touches many lives beyond those directly involved in the accident," Col. Ronald Fizer, Dugway's commander, said in a statement.

He said the base is providing support to all those affected.
