BOSTON (AP) - State Public Health Commissioner Cheryl Bartlett will be joining Cape Cod Healthcare to form a substance abuse and treatment program.

Bartlett said in a statement Tuesday that she will leave the department in mid-December, a few weeks before the end of Gov. Deval Patrick’s administration.

A registered nurse, Bartlett became commissioner in June 2013 after previously serving as a deputy commissioner and as director of the Bureau of Community Health and Prevention.

At Cape Cod Healthcare, Bartlett said she will be creating an entirely new program to serve residents of Cape Cod and the Islands.

Secretary of Health and Human Services John Polanowicz praised Bartlett’s tenure with the department, including her work in addressing the opioid overdose epidemic and an initiative to promote healthier lifestyles in Massachusetts.
