MEDFORD, Ore. (AP) - An investigation has determined that a woman who died with her 6-year-old daughter in a crash was at fault in the Butte Falls Highway collision.

The Mail Tribune reports (https://bit.ly/1Esxmtr ) the Jackson County District Attorney’s Office issued a release Thursday stating that 34-year-old Amy Sue Foster would likely have faced criminal charges if she had survived the April 2 crash.

The crash occurred after police received a report of a minivan driving erratically as it headed west. The DA’s office found that Foster, the driver of the minivan, had failed to negotiate a bend in the road and was driving too fast when she struck Jerey Kuchera, who had been driving a log truck in the eastbound lane.

Kuchera survived, and three children are still recovering from the crash.
