DENVER (AP) - Colorado Medicaid officials say they mistakenly mailed some people’s protected health information to the wrong address.

KUSA-TV reports (https://on9news.tv/1LfM4J7 ) that information belonging to 1,622 households was incorrectly mailed to other people between May 25 and July 5.

The department says none of the letters had Social Security numbers or other financial information. However, the letters may have included names, addresses, state identification or Medicaid case numbers, among other information.

The state has contacted those affected asking that the letters be returned or destroyed.
