GREEN COVE SPRINGS, Fla. (AP) - Five people died and two young children survived a head-on collision near Jacksonville on Tuesday.

The Florida Highway Patrol officials say a Toyota Camry heading north on State Road 21 in Green Cove Springs crossed over the center line and hit an oncoming car driven by 22-year-old Chelsea Johnson.

Family members said Johnson was on her way home from her husband’s graduation from the police academy in Orange Park when the crash occurred. Troopers said Johnson died at the scene.

Troopers said the driver of the Toyota, Daniel P. Eden Jr., 29, and passenger Brandy R. Eden, 24, both of Sisterville, West Virginia, died at the scene along with passenger David Neal Eden of St. Augustine. A fourth passenger, Rebecca Suzanne Bates, 46, of Keystone Heights, was taken to Orange Park Medical Center where she was declared dead.

Troopers said Daniel P. Eden III, 4, and Madison Eden, 2, also of Sisterville, West Virginia, were listed in critical but stable condition at UF Health Jacksonville. The incident report indicates that the older child was not wearing a seatbelt or sitting in a car seat.
