PORT ORANGE, Fla. (AP) - State officials have ordered a Port Orange assisted living facility to close after failing to report incidents of sexual assault.

The Daytona Beach News-Journal (https://bit.ly/2abth5v ) reports that the Agency for Health Care Administration ordered Grace Manor Assisted Living and Memory Care to close its doors by July 26.

Officials say the order stemmed from an incident in which a male resident was found in a room with an unclothed female resident with “cognitive deficits.” The order states that there were at least three instances where a male patient attempted to engage in sexual acts with female residents.

Grace Manor couldn’t be reached for comment by the newspaper.

Agency for Health Care Administration spokeswoman Shelisha Coleman says Grace Manor can appeal the decision within 30 days.
