BALTIMORE (AP) — An independent investigation into the death of a Baltimore man who died while in police custody last year cleared the officers involved of use of excessive force, but revealed many tactical errors by the police department.

Baltimore Police on Friday released a report published by an independent review board into the death of Tyrone West.

Baltimore police encountered West during a traffic stop on July 18, 2013. Police say when officers discovered drugs on West, a struggle ensued. After the officers were able to force West onto the ground and handcuff him, he stopped breathing.

The report says West died of cardiac arrhythmia.

The report says the death was sudden and not the result of force, but says the department broke police protocol several times.
