RICHMOND, Va. (AP) - The Virginia Board of Pharmacy has revoked a Richmond pharmacy’s permit and a pharmacist’s license because of safety concerns.
Westbury Pharmacy also will pay a $65,050 penalty. Former pharmacist-in-charge Faiz A. Oley Jr. will pay a $55,050 penalty.
The board on Friday released consent orders in which Westbury and Oley agreed to the penalties.
The consent orders say Westbury and Oley failed to take steps to prevent the diversion of controlled substances. More than 47,000 tablets of oxycodone were lost between May 2012 and and July 29, 2014. Smaller amounts of other controlled substances also were lost during the period.
According to the orders, the losses were in part due to theft by an employee.
Other violations included expired drugs that were mixed in with the pharmacy’s drug stock.
