GRAND JUNCTION, Colo. (AP) - Pilot error likely caused the 2013 crash of a small plane in western Colorado that killed two passengers and the pilot.

The Daily Sentinel in Grand Junction reports (https://bit.ly/1R8jUmS ) that the National Transportation Safety Board released a report saying the pilot, 49-year-old Dana Brewer, failed to maintain adequate airspeed while flying at a low altitude, causing the December 2013 wreck.

The wreck killed Brewer and her two passengers, 47-year-old William Kennon and 47-year-old Richard Platz, both residents of the Western Slope.

The plane apparently stalled and went into a spin before crashing in a remote, snow-covered area about 12 miles east of Collbran.

A toxicology test shows Brewer consumed marijuana prior to the accident, but there was not enough evidence to determine whether she was impaired by the drug.
