OTISCO TOWNSHIP, Mich. (AP) - Authorities say a 62-year-old man changing light bulbs at a western Michigan fruit storage facility died after falling about 20 feet.

Ionia County sheriff’s officials say Arturo “Sam” Medina died Saturday morning at Belding Fruit Storage in Otisco Township. The 38-year employee was standing in an apple crate while a co-worker raised him in a forklift, and investigators say he fell after rotted wood at the crate’s bottom gave way.

Authorities say Medina wasn’t wearing a safety harness or hard hat.

Michigan Occupational Safety and Health Administration officials are investigating the accident.
