BURLINGTON, Vt. (AP) - A regional nursing association is partnering with the University of Vermont Health Network in hopes of providing more seamless and personalized care while reducing unnecessary hospitalizations and controlling costs.

The Burlington Free Press reports that the Visiting Nurse Association of Chittenden and Grande Isle Counties will join the UVM network on Jan. 1 after a unanimous vote of its board of directors. The two organizations have been discussing ways to improve health care since 2015.

Hospital officials say having the Visiting Nurse Association join the network would allow the hospital to better coordinate patient care. Officials with the nursing group say the partnership will allow it to expand telemonitoring services and increase educational opportunities for clinical staff.
